Chapter 4 - The Diagnosis and the Dossier

“Are you David Vance?” Dr. Aris asked, pulling off his surgical mask.
“Yes. Doctor, how are my wife and son?” David stood up immediately, putting his laptop away.
Dr. Aris gestured toward a private consultation room. “Step inside, Mr. Vance. We need to talk before I file my mandatory report.”
David’s heart sank into a dark abyss. “Mandatory report?”
“Your wife, Maya, has a severe internal surgical site infection,” Dr. Aris said directly. “Several of her deep fascial sutures have failed due to trauma and physical exertion. She has early-stage sepsis caused by untreated bacterial exposure, combined with acute malnutrition and dehydration. If you had brought her in twelve hours later, she would have gone into septic shock.”
David closed his eyes, his chest rising and falling heavily. “And my son?”
“Liam is suffering from mild hypothermia and severe gastrointestinal distress caused by inappropriate nutritional intake. His weight has dropped eight percent since his birth record. Someone has been feeding him low-grade, non-hydrolyzed formula that his digestive tract cannot absorb.”
Dr. Aris leaned across the table, his eyes piercing. “Mr. Vance, as healthcare providers, we are legally mandated to report suspected adult dependent abuse and severe infant neglect to the NYPD and Child Protective Services. Your wife told the intake nurse that she was locked in the apartment without fresh water or adequate heat for three days while recovering from surgery.”
David pulled out the USB drive and set it firmly on the table between them.
“I am not the abuser, Doctor,” David said, his voice terrifyingly calm. “I was out of the country on business. I wired fourteen thousand dollars to my mother and sister to care for my wife. They took the money, stole the medical formula, physically assaulted my wife, turned off the heat, and went on a luxury trip to Miami.”
Dr. Aris stared at the USB drive in disbelief. “You have proof?”
“I have three hundred hours of 4K security footage with clear audio,” David said. “Call the NYPD. I want a detective here right now.”
At that moment, David’s phone rang.
The screen read: Eleanor (Mom) - 14 Missed Calls.
David answered, putting the phone on speaker mode so Dr. Aris and the head nurse could hear.
“DAVID!” Eleanor’s hysterical voice shrieked through the speaker, competing with the distant sound of Miami police sirens. “WHAT IS THE MEANING OF THIS?! OUR CARDS ARE DECLINED! THE HOTEL MANAGEMENT IS THREATENING TO ARREST US! WE ARE STRANDED AT THE FRONT DESK OF THE ROYAL PALM!”
David did not raise his voice. He kept it at a steady, freezing tone.
“Are you having a nice New Year’s Eve, Eleanor?”
“DON’T YOU 'ELEANOR' ME!” she screamed. “I AM YOUR MOTHER! MARK WAS ALMOST TAKEN INTO CUSTODY BECAUSE OF THAT UNPAID BAR TAB! UNLOCK THE CARDS IMMEDIATELY AND WIRE FIVE THOUSAND DOLLARS TO MY VENMO!”
“Did Maya enjoy her instant ramen?” David asked quietly.
A brief, suffocating silence fell on the other end of the line.
“What... what are you talking about?” Eleanor’s voice faltered slightly. “You’re supposed to be in Germany!”
“I came home, Eleanor,” David said. “I walked into my freezing home at 11:30 PM. I saw my wife bleeding on a plastic chair. I saw my son blue from the cold. And then... I checked the cameras.”
Eleanor gasped loudly.
“David... son, listen to me,” Eleanor’s tone shifted instantly from rage to desperate, oily panic. “Maya is lying to you! She’s mentally unstable! Postpartum depression! She threw the food away herself! She made a scene because she hates us!”
“I watched Mark shove her into the table on December 28th at 11:14 AM,” David said calmly. “I watched Chloe toss her coat on her open incision. I watched you turn the thermostat down to 62 degrees.”
“David, please!” Chloe’s screeching voice chimed in from the background. “We’re your family! You can’t leave us stranded in Miami with no money! How are we supposed to get home? How are we supposed to pay for the hotel?!”
“Sell your designer dresses,” David said. “Pawn your earrings. Sleep on the beach for all I care. Because if any of you set foot near my apartment again, you won't be dealing with me. You’ll be dealing with the NYPD.”

David hung up the phone and turned it to Do Not Disturb.
He looked at Dr. Aris. “Doctor, please bring the police in. I’m pressing full charges.”
